Youth Sports Coaching Application

All youth sports coaches in Broadview Heights are required to submit a coaching application prior to being selected to coach a youth sports team. 

This application contains the following items:

1. General Information about the coach such as name and experience.
2. A background check consent form. The background check will cover a criminal background check, sex offender registry check, address verification and social security verification.
3. A list of recommended criteria for exclusion from coaching based upon the background check results.
4. Information about how to obtain the concussion training certification mandated by the State of Ohio for all youth sports coaches.
5. A copy of our Coaches Pledge which is to be signed.
6. A copy of our Parent Code of Conduct
7. A copy of our Zero Tolerance Policy
