The undersigned does hereby waive, release, and hold harmless and indemnify the City of Broadview Heights and the Broadview Heights Parks and Recreation Department, their organizers, officers, employees, agents, and sponsors for any and all claims for damage of personal injury to me or my child or loss of property that may be cause by any act or failure to act on the part of the City of Broadview Heights, the Broadview Heights Parks and Recreation Department, their organizers, officer, employees, agents and sponsors. The undersigned further assumes the risk of all dangerous conditions in and about the City of Broadview Heights, the Broadview Parks and Recreation Department property both real and personal and waive any and all specific notice of the existence of such dangerous conditions, if any. By participating in or attending any Parks and Recreation events I agree to allow publication of any photograph taken of myself/my child for advertising purposes.
*By entering the building all patrons agree to adhere to the current rules and policies of the recreation center. These rules can be found online at www.bhrec.org or by request at the front desk.
I hereby certify that I am the adult parent or guardian of the minor entered in this waiver, a minor child under the age of eighteen years, and I consent to his/her participation in recreational activities with the Broadview Heights Recreation Center. I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage) of (1) said minor child’s participation in recreational activities the Broadview Heights Recreation Center. I recognize my responsibility to ensure that said minor child participates only in any activities for which he/she has the required skills, qualifications, training and physical conditioning and will follow all posted and online rules of the facility. I understand that the City of Broadview Heights shall have no responsibility to pay for medical treatment and related costs if said minor child is injured. I am at least eighteen years of age and have carefully read and freely digitally signed this and the above general Liability Waiver and Release Form (Minor Child). I understand and agree that no oral or written representations can or will alter the contents of this document. I understand this document to be valid until the minor child is no longer a minor.
