Pandemic Survey

PANDEMIC SURVEY 

The Broadview Heights Human Services Department is working with the City of Brecksville Human Services Department and CAPA (Community Awareness Prevention Association) on conducting a survey engaging older adults 50+ during a pandemic.  If you agree to help with the survey, you will answer a 15-minute survey that asks about your experiences during the pandemic and about the services you have used or you have been unable to receive.  Your answers will be anonymous; please do not write your name on the survey.  A link to this survey will also be available to take on Survey Monkey.  

Your answers will be used to improve current services and to develop new services.  The group report will be provided to the City of Broadview Heights and Mayor Alai.   There is no risk for you to participate.   If you need help now or if you have any questions about this survey, you can call Amy Washabaugh or Wade Cramer at 440-526-4074. Please complete the survey by Friday, November 27, 2020.
